Procedure |
Policy: When Community Care desires to classify protected health information as "de-identified" or "re-identified" it may only do so after direct consultation with a person with appropriate knowledge of and experience with generally accepted statistical and scientific principles and methods for rendering information individually identifiable AND who is well versed in the requirements set forth in the Privacy Rule at 164.514 a, b, and c.
