Policy:  Community Care may deny an individual access without providing the individual an opportunity for review if the client has requested psychotherapy notes; information has been compiled in reasonable anticipation of, or for use in, a civil, criminal, or administrative action proceeding; Community Care is a health care provider acting under the direction of a correctional institution and is denying access to an inmate and such request would jeopardize the health, safety, security, custody, or rehabilitation of the individual, other inmates, the safety of any officer, employee, or other person at the correctional institution; access that is governed by the Privacy Act; if the information was obtained from someone other than a health care provider under a promise of confidentiality and the access would be reasonably likely to reveal the source of the information.
