Policy: Whenever there is a material change to the uses and disclosures, the individual’s rights, the covered entities legal duties, or other privacy practices stated in the privacy notice regarding protected health information, Community Care will promptly revise and distribute it’s notice. Except when required by law, a material change to any term of the notice may not be implemented prior to the effective date of the notice in which the material change is reflected.
