Community Care will
provide persons with disabilities an opportunity to participate or benefit from
all services as provided or offered to other individuals.� This will be done in the most integrated
setting appropriate to the needs of the individual with a disability.� Community Care will not use standards,
criteria, or methods of administering services or facilities that screen out,
exclude, or discriminate based on disability.

Community Care will make
any reasonable modification to policies, procedures, or practices to ensure
equal opportunity to all persons to the facilities and services we offer.� All structures, communication avenues, and
alterations as needed will comply fully with all accessibility
requirements.� Auxiliary aids and services
will be provided at no cost by Community Care as appropriate to any individual
and as required by the Americans with Disabilities Act.

Community Care will not
discriminate against any person who is related to or associated with a person
with a disability.
4.
Any public event
sponsored or hosted by Community Care held outside of a Community Care facility
will be held in a facility which is accessible to persons with disabilities.
5.
Community Care will employ
a telephone device for the deaf (TDD) in each of its administrative offices and
will display this fact and phone number in all its relevant published
literature.
