Procedure: The attached document shall serve as the offcial notice of Privacy Practices for Community Care. Changes may not be made without prior approval. All requests to revise the Notice must be made through the Privacy Officer. The Privacy Officer will work with the Maine Management Team to approve the revisions. In the event a revision is made, all old copies of the Notice will be destroyed and replaced with the revised Notice. The Notice will be amended to clearly indicate the effective date of the new Notice. The new Notice will be incorprated into the pre-service training. The Privacy Officer will notify the MIS Dept about the change and a copy of the revised Notice will be placed on the website. All providers will receive a copy of the revised Notice.
