Policy: It shall be the policy of Community Care that all providers, student interns, and consultants document all services provided to clients in a timely, accurate, and orderly fashion. This documentation will include progress toward service/treatment plan goals, as well as other interactions that may be pertinent to the service/treatment needs of the client.
