Policy: It shall be the policy of Community Care that information regarding clients or their contact with Community Care shall be kept in complete confidence. No information shall move beyond Community Care’s providers without the client's (or guardian as appropriate) informed and written consent except as required by State or Federal Statute. Provision for the protection of the client's right to privacy among providers shall be paramount and information shall be shared only for clinical/treatment purposes within the case coordination process. Release of information by a provider (other than in response to legal mandates) without the client's (or guardian as appropriate) informed and written consent shall be considered immediate grounds for discharge. Clients and guardians shall be informed concerning the types of records kept, the way in which they are used by Community Care, his/her right to access to his/her records, and the disposition of the record after (s)he leaves the program.
