Policy: Community Care recognizes the importance of various assessments throughout the provision of services to clients. Assessment types may include, but are not limited to: a. Intake or Screening Assessments, b. Comprehensive Basic Assessments, c. Comprehensive Psychosocial Assessments, d. Mental Status Assessments e. Risk Assessments. Information from assessments will be used to develop treatment/support plans that are specific to the needs and strengths identified in the assessments. Information from assessments will also guide treatment teams as they provide program services. Ongoing assessments will be completed to determine areas where the client has made progress and areas that still need to be addressed. Service delivery will be adjusted accordingly, with the involvement and approval of the client and/or their legal guardian.
