Individualized Case Record |
Procedure |
Policy: It shall be the policy of Community Care that an individualized case record will be maintained for every client. All members of treatment groups will have their own personalized case records and confidentiality for group members will be maintained within individual case records. Client records shall contain identifying information sufficient to describe the client’s background, resources, need for treatment, and documentation of service delivery.
