Procedure |
Policy: It is the policy of Community Care that when a client's third-party benefits or payments end, the agency will extend up to three (3) free clinical sessions to the client, (if clinically assessed as necessary), while connecting him/her to available community resources in order to ensure as smooth a transition from services as possible for the client. In all circumstances, as documented in the client's discharge summary, Community Care will determine the need to ensure the safety of clients who are, or may be, in crisis. In such cases, discharge planning will include referrals to available community crisis support programs.
In all circumstances Community Care will determine, by completing an After Care Checklist, the need to ensure the safety of clients who are, or may be, in crisis, including making referrals to available community crisis support programs.
