If services are provided
by a contractee or sub-contractor, a documented affiliated service agreement
will exist.

Contractees and
sub-contractors providing service for clients will be considered members of the
Treatment Team and will be expected to work collaboratively with the Team to
ensure consistent quality care for the client.
3.
In all cases of
transition of one service to another, all providers will ensure a collaborative
planning process and provision of services that are sensitive to the client�s
transition needs.
4.
Community Care clients
will be referred to contracted service providers only after a determination of
need has been made by the Treatment Team and documented in the client�s service
plan.
