Involvement of Families of Origin, Kin, and Other Significant Persons in the Life of a Client

Procedure:  .

1. Employees completing intake referral/screening will:

  1. Inform the referring party that Community Care encourages participation of family of origin, - extended family, and other significant persons when assessing, planning, delivering, and evaluating services to clients when such involvement is not contraindicated;
  2. . Actively solicit from the client, as appropriate, names of specific individuals the client wants asparticipants in planning and service delivery.
  3. Inquire about the status of parental rights (e.g. PPO, C-1, C-2, Cease Reunification, TPR etc.)
  4. d. Acquire additional relevant information regarding the status of parental rights.

2. Service provider, or designee, will consult with the client�s legal guardian with in 24 hours of case assignment, emphasizing the importance of including the family of origin, kin, and/or other significant persons in the client�s initial assessment, subsequent services, and permanency planning process. This may include, but is not limited to, the visitation agreement for supervised visits, parameters for including Family of Origin, Kin, and/or Other Significant Persons in the initial and subsequent team meetings (i.e. frequency, duration, transportation to meetings, special conditions/considerations of family attendance, etc.). Consultation with the referring party may be by phone, or when possible, in person so that the necessary releases to move forward can be obtained.

3. Discussion to involve or not involve Family of Origin, Kin, and/or Other Significant Persons must be initially documented on the service provider�s contact notes or on the referral form and place in the case record with 24 hrs. Service plans must indicate the involvement or exclusion of Family of Origin, Kin, and/or Other Significant Persons. When excluded, reasons must be clearly documented on the Service Plan. This information is to be reviewed at least quarterly.

4. When consent for Family of Origin, Kin, and/or Other Significant Person�s participation is granted, and authorizations to disclose and/or receive information are signed, the service provider will contact individuals by phone, followed by written notice inviting their participation.
