3.ME.25 Maintaining Contact or Communication with Family and/or Significant Other Persons Policy

Procedure:  .

Community Care recognizes the potential benefit that clients may experience in maintaining connections with their family and/or other significant persons. Community Care will ensure that the client's visitation rights are upheld unless contraindicated.

  1. At the time of intake, visitation will be assessed. Prior to the development of the preliminary service plan, further discussion will include the client, service providers, and the legal guardian. These discussions and decisions will be documented in the case record.
  2. A visitation agreement will be completed by the client's guardian that will include: who may visit;frequency, conditions and location of the visits; and all other related guidelines. The client�s age and developmental stage, family strengths and needs, visit participants, the social and cultural context of the family, and the permanency goal will also be considered in establishing a visitation plan. This information will be documented in the client's case record. Guidelines for visits will be explained to all visitors by the visit supervisor or designee and will be placed in the case record. In situations when the visits are held in the home of the parent(s) or a relative, the service provider or designee will complete the documentation and team process required for in-home visitation.
  3. The provider responsible for provision of the visit will offer guidance and support to participants before and after visits, assess interactions and skills during visitation, and document their observations.
  4. Visits will not be cancelled as a disciplinary action.

5.��� If it is decided by the team that visitation is contraindicated, the reasons will be documented in the service plan and in the case record. This decision will be documented, signed off by the client's team, including the guardian, and then reviewed by the team every 6 months minimally. Bottom of Form
