3.ME.19 Assessment Policy

Procedure:  t the time of intake Community Care will identify any special needs known for the client being referred. 1. Within 30 days of admission an initial assessment will be conducted for all clients. 2. This assessment will be conducted by a licensed clinician. 3. In addition to the appropriate clinical assessment process, the assessment will include, as appropriate: a. relevant health and safety issues, b. the degree of supervision needed, c. independent living skills and skills in the activities of daily living, d. nutritional and dietary needs, e. leisure interests, aptitudes, and the need for greater social inclusion, and f. other services and resources. 4. When appropriate to the needs of the person served, the assessment includes a review of adaptive behavior and cognitive function, including concrete and abstract reasoning and intelligence. 5. When appropriate to the needs of the person served, the assessment addresses the need for assistive technology, auxilliary aids and services, and other special accommodations. 6. When appropriate to the needs of the person served, the organization conducts a vocational assessment. The vocational assessment will include: a. work history, b. skills, c. interests, d. experience, e. education, f. prior training or need for training, g. related behavioral issues, and h. the need for accommodations. 7. Whenever any part of an assessment deemed appropriate for a client cannot be carried out by the Community Care employee, a referral will be made, in conjunction with the legal guardian, to an appropriate external resource in order to complete the assessment.
