Policy |
Procedure: Community Care will maintain service continuity for clients by avoiding the arbitrary or indiscriminate reassignment of direct service personnel and, when appropriate, by assigning providers at intake. 1. Community Care will assign a service provider to each client at the time of intake. 2. The service provider will discuss with the treatment team the client's need for other direct service providers and these conversations will be documented in the client's case record. Direct care providers (internal and external) will be added to the service plan and entered into the case record. 3. When a direct service provider is reassigned, the team will meet and discuss the best interest of the client and plan for the least disruptive transition from the client's and/or family's perspective. When possible, the transition will include a meeting to ensure that the client and/or family have an opportunity to meet the new provider and discuss treatment needs. Changes in the service plan will be documented in the case record.
