Procedure: The psychosocial assessment will be completed within 20 days of Community Care receiving a referral.
The Support Worker assigned to the child's case will complete the assessment with the child and with his or her guardian. Other service providers' input will be included as needed.
The Support worker will meet with the child face to face in their living environment in order to interview the child and complete the assessment.
Once completed, the written assessment will be placed in the client's file and a copy will be sent to the guardian.
The assessment will be updated annually, which will include all progress relating to service plan goals. The assessment must be updated within five working days of:
A. a precipitating event (ie. child runs away and returns to foster home)
B. whenever a change occures in the status or situation of the client or with
his/her caregivers (ie. child is transitioning to an adoptive home)
C. another problem occurs (ie. allegation made against the child)
