When it is necessary for a client to transition a new
home within the Treatment Foster Care program, the following preparations must
be taken:
1. Advance notice prior to a placement move will be
given to allow for appropriate planning;
2. If possible, alternative foster parents that have
the skills and characteristics necessary to meet the needs of the client will
be identified;
3. Specific client information will be provided to the
prospective foster family;
4. Visits, activities, and at least one overnight
visit should take place prior to the move;
�5. If needed,
additional supports will �be put in place
to reduce the amount of distress/trauma experienced by the client during the
transition;
6. Provisions will be made to ensure that the client
is able to maintain contacts with the previous foster family or other social
connections, if considered appropriate by the team, including the guardian;
�7 All clients
must be allowed to keep personal items areimportant
to them. The process of any transition must be documented in the client's case
record. In some situations, circumstances may not allow for the one or all of
the above steps to take place during a transition from one home to another. In
those circumstances the reasons must be clearly documented and placed in the
client's case record.
