Procedure: 1.) When a support worker is assigned to a client by the Program Coordinator, (s)he will begin the intake process by contacting and meeting with the client/family within 7 working days (depending on the availability of the client/family). Prior to meeting with the client, the support worker will complete the client information form and submit for pre-authorization. Intake process will be done in collaboration with other providers assigned to the client, and will involve:
a. Initial introductions
b. Education on the services that are available through the Family and Community Support Services Program
c. Discussion of initial services/assessment/service planning procedures
d. Discussion of client/family and agency expectations
e. Reviewing the mandated reporting laws
f. Reviewing rights of recipients of individuals needing mental health services
g. Notification of rights
h. Review notice of privacy practices
i. Completing of initial releases
j. Completion of the Preliminary Service Plan (including discussion of immediate services required)
k. Scheduling of initial assessment
2.) If the child has a legal guardian assigned by the state and/or a case manager assigned, the support worker will collaborate with these providers throughout the intake process
3.) The referral information and information gathered at intake will be forwarded to the Clinical Supervisor to be utilized in the assessment process.
