3.ME.2 Access to Services Policy

Procedure:  1.) Family and Community Support Services will receive referrals from case management agencies, BDS, birth/kinship/adoptive families, or self-referrals for individuals who are 18 years of age or older. Referrals can be made over the phone or in person by appointment. 2.) All referrals will be received by the Program Coordinator who together with the referring party will determine eligibility for services based on the program’s service eligibility criteria (please see program description for further details). Prior evaluations may be necessary to determine eligibility. 3.) If the individual is not found to meet the program’s service eligibility criteria to receive services, (s)he will be referred to other community resources. In situations that the individual does have a case manager, Community Care Program Coordinator will contact the case manager to inform them that the individual does not meet the program’s service eligibility criteria at this time. 4.) Individuals who do meet eligibility criteria but services are not available, will be referred to other community resources. The individual will be given the opportunity to be placed on the waiting list. Children/families will be contacted each month regarding their status on the waiting list. 5.) From initial contact, eligibility determination will be completed within 2 business days depending on the availability of the individual referring. 6.) For those individuals who do meet the program’s service eligibility criteria, the Program Coordinator will assign a support worker to the client/family to begin the intake process. Program Coordinator will contact the Clinical Supervisor who will work in collaboration with the Support Worker to schedule the client’s initial assessment.
