Policy |
Procedure: The CFSS support worker will work with the family to assess their need for resources, and provide hands on assistance in role modeling and educating the family on seeking resources. Support offered to the family will include seeking identified resource needs. CFSS will develop and maintain a close partnership and collaboration with local case management agencies. The high level of collaboration will allow for an efficient process of supporting families in referring to a case management agency when there a need for coordinated care on behalf of the child that exceeds the limitations of family/support worker.
