Policy: It shall be the policy of Community Care that each client will be provided with a continuum of care that has a minimum disruption. Community Care will work closely with the placing agency and/or guardian, clinician, foster parent, and other providers as necessary to minimize disruption and maximize quality of care. The supervision for quality of care is under the guidance of the client's Community Care service provider and the service provider's supervisor.
