1)
The formal process of completing
evaluation forms will take place 180 days after the date of hire (coinciding
with the end of the six month probationary period) with a 6 month sign off form. Upon 1 year of service from hire
date, there will be a formal evaluation completed and every year hence. The
annual evaluation will be due on the provider's anniversary date.

The provider's supervisor will be
responsible to track evaluation dates and for initiating the

Performance Review and completing
the evaluation form in a timely manner.

The evaluation form will be
reviewed by and approved by the appropriate Program Director.

The Performance Review process is
to be conducted in a manner as to:
i)
Encourage thoughtful consideration
of a provider's performance based on reasonable performance expectations.
ii)
Improve each provider's performance
through performance feedback and identifying provider goals.
iii)
Improve each provider's job
satisfaction through positive reinforcement and constructive comment.
iv)
Serve as a guide for supervisors in
assisting providers in planning for professional growth and development.
v)
Assemble data for use as a guide
for such purposes as salary increases, promotions, disciplinary action, and
reassignment.

Completed and approved evaluation
forms will be signed by the provider. The signed evaluation

form will signify that the provider
has reviewed, not necessarily agreed with, the evaluation. The provider will
receive the original evaluation form with copies going to the provider's
supervisor and personnel record.
8)
If the provider wishes to comment
on the performance appraisal process or the evaluation form, (s) he may comment
in writing within a reasonable period of time not to exceed five (5) working
days. This may be done directly on the evaluation form, or separately as
desired. This material will be reviewed by the Human Resources Manager or
designee and will be attached to the evaluation form in the personnel file.
