Procedure:  In developing this program, all sites shall consider the following: 1. A review and approval of each plan by the Team Clinical Supervisor or designee. Specific protocols to be considered in the approval of such plans include. · Establishment of crisis assessment and response teams to include all interested stakeholders. · Maintenance of information on incidents of violence for review and analysis. · Establishment of a procedure for employees to confidentially report threatening situations and other relevant information to management. · Establishment of a procedure of when to contact emergency response authorities. 2. An awareness program for workplace violence prevention should include initial and refresher awareness and response orientation and instruction offered to all employees. The Health and Safety Committee will provide guidance for implementation of this program for employees. 3. Written communication from each site of all incidents of violence and subsequent outcomes as the incident occurs to the Safety Coordinator. Written communication from each program of all incidents of violence involving employees and subsequent outcomes to Human Resources. 4. An annual review of all workplace violence incidents and outcomes submitted to the Safety Committee.
