Procedure: 1. At various times Community Care clients may participate in special program sponsored activities such as bicycling, overnight camping and backpacking in a non-remote area, day hiking, low-risk canoeing/kayaking/boating, other low-risk outdoor activities. Any time such activities are planned Community Care must evaluate persons served for their ability to participate in these activities.
2. The follow must be obtained prior to engaging a person served in these special program activities:
· A written, signed permission slip/form from the participant's legal guardian,
· A medical records release/authorization for disclosure signed by the client and/or their legal guardian, and
A signed document from a medical professional stating that the participant is physically capable of participating.
