Policy: Protected Health Information of clients receiving services within the agency may not be used or disclosed for purposes other than as identified or required by policies and procedures of the agency. Protected Health Information is defined as: individually identifiable health information. Examples include, but are not limited to, all information regarding a client's intake and assessment, diagnosis(es), treatment planning, progress in treatment, contact with professionals, medications, appointments, attended therapy session dates, and all information within the client record.
