1.MEP.9 Record of Disclosures of Protected Health Information Procedure

Policy:  Community Care will keep a record of disclosures of protected health information as part of the client chart. The record must include the date of disclosure; the name and address of the entity or person who received the protected health information; a brief description of the protected health information disclosed; a brief statement of the disclosure that reasonably informs the individual of the basis for the disclosure. Please Note: Not ALL Disclosures must be recorded. The following disclosures are EXCLUDED from the above requirement. Disclosures: To carry out treatment, payment and health care operations; to clients who have requested access; incident to a use or disclosure per Community Care Policy 162:506(c); Pursuant to an authorization; to correctional institutions or law enforcement officials; for the entity’s directory or to person’s involved in the individual’s care; for national security or intelligence purposes; that occurred prior to 4/13/03.
