Policy: It shall be the policy of Community Care that all individuals served by Community Care shall have a written individual plan of care. All plans will be developed and reviewed in a timely manner and will be based on the initial and ongoing assessments. Service plans will be developed and reviewed in conjunction with the service providers and will fully involve clients, parents, and/or guardians to the greatest extent possible. The service plan will be the basis for delivery of appropriate services and support.
