3.ME.1 Continuity of Care Policy

Procedure:  .

1.        Community Care assumes case management/coordination services for clients when one or more of the following conditions exist:

o        The need for case coordination has been identified;

o        No other organization has assumed that responsibility;

o        The type of service, or lack of service coordination by multiple providers necessitates that the organization assume a central coordinating role;

o        Community Care is contracted to provide the service;

o        Community Care is not contractually prohibited from doing so; and/or

o        Community Care is the most appropriate entity to assume such a role.

 

When appropriate Community Care will attempt to partner with individuals and/or families who are in need of service coordination when it is not being provided by another organization.

 

2. Services may include but are not limited to.

 

o        Coordination of services between providers and/or agencies;

o        Establishment of communication between a team of providers;

o        Ensuring the documentation of the team process;

o        Ensuring that documentation is collected and placed in a client case record;

o        Ensuring the creation and implementation of the client's service plans;

o        Ensuring the protection and confidentiality of the client's case record;

o        Assuring the provision of adequate support to the client throughout the service provision;

o        Assuring appropriate planning for services; and

o       Assuring appropriate discharge, termination, and aftercare planning.
