3.ME.30 Continuation of Services When Benefits Have Ended Policy

Procedure:  .

At the point when it becomes known that a client's benefits or payment for services is going to, or has, run out, Community Care will engage in the following procedure.

 

1.        Communicate the situation to the program supervisor

2.        Communicate the situation to the client and/or the client's guardian

3.        Complete an Discharge Summary/Aftercare Services Checklist

4.        Evaluate the need for the client to receive up to three (3) free clinical sessions

5.      Connect the client with available community resources to maintain safety and support during the transition from services at Community Care

6.       Upon completion of the above procedures and when the client case is closed, a discharge summary will be entered into the client's record on the day of discharge.
