Procedure: Upon completion of the initial assessment by the clinician or their designee, questions will be asked of the youth and /or family to assess level of risk and concerns for safety. Information will also be gathered on reason for referral, goals of in home support services, and barriers to accessing needed services to address areas of identified need.
Monthly thereafter, support workers and/or clinician will complete the ongoing risk/safety assessment by asking the family a series of questions. The risk/safety assessment questions are included on the progress note form, instructing the support workers and/or clinician to complete the questions with the family monthly. If any of the questions are answered in the affirmative (indicating potential risk/safety concerns), the support worker and/or clinician will forward the progress note to the clinical supervisor and will follow up with the clinical supervisor as indicated.
