Procedure: When Community Care determines that a Foster Parent requires access to the data network or a change to their access is required, the following will occur: The Team Area Manager will complete the Information Access Request Form; if the request is for a Respite Parent it will be forwarded to the Treatment Foster Care Manager for evaluation prior to the next step; otherwise the request will be routed to the IT Department for action. The IT Department will complete the action as requested and notify the Team Area Manager that the account is ready for use. Each submission of the Information Access Request Form will supercede previous requests.
