Policy: It shall be the policy of Community Care that the performance of all providers be reviewed,
evaluated, and discussed on an on-going consistent basis. Performance will be based on how the
provider performs the duties of the job as stated in the provider’s job description and how the
provider performs in relation to specific performance guidelines addressed in previous performance
evaluations, Professional Development Plan, corrective action plans, or during supervision
opportunities.
