Procedure: 1. Upon receipt of a provider's letter of resignation or notice of dismissal, the Human Resources Coordinator or designee will send a letter to the provider offering the person the opportunity to schedule an exit interview with the Human Resources Coordinator, the person's supervisor (or designee), or a team representative.
2. Those facilitating the exit interview will use a standardized set of questions.
3. Information gathered during the exit interview will be held in the strictest confidence.
